External fixation is an integral part of orthopedic treatment in trauma deformity correction. Since its first use by Dr. Raoul Hofmann in modern surgery in 1938, & which was revolutionized by Dr. Gavriil Illizarov in the 1950’s by introducing a new method of treating fractures, deformities and bone defects. Illizarov’s method is still standard of care even after 70 years of its first introduction. However, it has been modernized significantly improving design, material of construction.

(B) External Fixation

Pin-site infection is one of the inevitable side effects associated in all generations or evaluations of external fixation. Pin site infection is defined as infection resulting into open wound made for insertion pin/wire of external fixator. There are several factors which affect the rate of pin site infection, for example frequency of dressing done at pin-site, antiseptic / dressing material used to clean wounds, and cover of antibiotics.

Even after considering in practical life, pin site infection is seen in almost 100% of cases. Out of which up to 4% results into serious infection like osteomyelitis.

CURRENT STANDARD

WIDE VARIATION IN STANDARD

  • Gauge dressing with antibacterial / antiseptic solutions.

  • Gauge dressing with –

(a.) Hydrophilic content in order to keep wounds wet i.e wax, hydrocolloid etc.

  • (b.) Antiseptic solutions i.e silver content, highly vary how they use different materials.

  • Most common way to retain dressing is to use vial caps which is crude but effective solution so far. However results are highly variable as it is difficult to maintain standard of practice.

  • Foam – support

BURDEN OF INFECTIONS

Research shows that infection related to surgical site costs are 200% compared to those without infection.

  • Apart from that, there is huge impact on morbidity & prolong disability due to complicated surgical outcome.

NEW STANDARD FOR EXTERNAL FIXATORS

DESIGN

Baćoma® is a result of exhaustive efforts from multidisciplinary team in order to reduce infection and hence burden on pin site infection in case of external fixation.

BENEFITS

  • Baćoma® – helps to maintain a clear pin-site.

  • Standard practice helps surgeon to keep pin-site wound management.

  • Better visibility – Transparent biocompatible material unique design helps to keep surveillance on pin-site wound.

  • Reduces hospital follow ups & dressings by maintaining a sterile field around the pin-site.

  • As per one of hypothesis of pin-site infection / necrosis is promoted by unnecessary movement of skin around pin site.

  • Baćoma® holds skin around pin. Hence, reduce pin-site exudates and necrosis.

Baćoma® helps to reduce frequency of less visit to clinic for dressing & follow up.

WASHABILITY

  • External fixation is one of most troublesome therapy in orthopedics because of its restriction in day to day life.

  • Baćoma® is completely water resistant & protects pin-site wound from water.

Hence, patients can take bath without fear of getting infection at the pin-site.

AVAILABLE SIZES

Baćoma® comes in three sizes :
